Tax Form Information

TAX FORM INFORMATION
Per IRS regulations, all original tax related forms for tax year 2011 will be mailed through the U.S. Postal Service 1st Class mail system to the current mailing address on file by January 31, 2012.

STUDENTS
If you are a student who was enrolled in a term that began in 2011, you will receive a 2011 1098-T tax form in the mail by January 31, 2012 showing tuition billed.

If you believe your address on record is incorrect, please complete the Tax Information Request form below and click submit to update your current address on file.

EMPLOYEES
2011 Form W-2 for employees will be mailed through the U.S. Postal Service 1st Class mail system by January 31, 2012. If you are a student employee or regular employee and are requesting a duplicate Form W-2 for wages earned, please complete and submit the Tax Information Request form below. Duplicate W-2 forms are NOT available online and can only be requested using the form below.
