Admission - Required Forms
There are a few different forms that need to be filled out in order to complete the application/financial aid process. You can simply download the forms you need, fill them out, and send them back to us via either fax or mail. To do so, you will need an Adobe Acrobat PDF reader. If you do not have Adobe's free software on your computer, you can download and install your free copy from Adobe's download page.
All forms can also be requested by calling 800-762-5960. If you have not yet applied to the university and want to do so, you can complete your Admission Application online or you can download a paper application. If you're beginning your financial aid process you will need to fill out a Free Application for Federal Student Aid (www.fafsa.ed.gov).
For Admission:
- General Admission Application - Application to enroll to the university (not needed if you already applied).
- Adults interested in attending our evening division should complete the Adult Admission Application.
- Returning students should complete the Return Admission Application.
- High School Transcript Request Form - Request form to acquire your High School Transcript.
- GED Transcript Request Form* - Request form to acquire your GED transcript if High School transcript is not available.
- College Transcript Request Form - Request form to acquire your transcripts from previous universities attended.
- Transfer of Credit Agreement - Agreement between RMU and you in regards to RMU's transfer of credit policy.
- Personal Statement - Supplemental form to add any personal information to your Application (e.g. past job experiences, resume, community services, etc.).
For Financial Aid:
- www.FAFSA.ed.gov - Free Application for Federal Student Aid online.
- Online Pre-Application Worksheet - Helpful document to assist you in filling out your online FAFSA.
- Independent Worksheet - Must be filled out by anyone who can claim independency.
- Dependent Worksheet - Must be filled out by anyone who can still be claimed as a dependent.
*Students who have taken their GED in Cook County, you need to download the following form: Cook County GED Request Form (English), Cook County GED Request Form (Spanish) or go to: www.cookcountyged.org.
